Abstract

The rapid digitization of healthcare systems has intensified the need for secure, scalable, and intelligent infrastructures capable of managing sensitive clinical data across distributed environments. This research proposes a secure digital health transformation framework integrating cloud data governance, artificial intelligence (AI), multi-cloud orchestration, and end-to-end data lineage tracking. The study synthesizes existing literature on cloud computing in healthcare, predictive analytics, and governance mechanisms to design a unified architectural model that ensures data integrity, regulatory compliance, and operational efficiency. The framework leverages AI-driven decision-making to enhance predictive healthcare insights while ensuring transparency and traceability of data flows across heterogeneous cloud environments. Drawing from prior studies on healthcare cloud adoption, data lineage systems, and secure multi-cloud storage strategies (Griebel et al.; Hussain; Kuo), the research identifies critical gaps in interoperability, auditability, and governance automation. The findings suggest that integrating structured lineage tracking with AI-enhanced governance significantly improves system accountability and security. Additionally, cloud-based open-source evaluation strategies demonstrate how scalable architectures can support real-world healthcare interventions (Mayfield et al.). The study concludes that a unified governance and orchestration framework is essential for achieving resilient, compliant, and intelligent digital healthcare ecosystems.
